Most safety programmes fail quietly. They launch with a workshop, produce a binder, and then lose the attention of the people whose habits they were meant to change. The programmes that hold are the ones wired into how work already gets scheduled.

We start by mapping the tasks that carry real consequence, not the tasks that generate the most paperwork. That list is usually shorter than anyone expects, and it moves the conversation away from compliance theatre toward the handful of moments where an intervention genuinely changes an outcome.

From there the roadmap is deliberately unglamorous: clear ownership for each high-consequence task, a single place where deviations get logged, and a weekly review short enough that supervisors actually attend it.

The technology matters less than the cadence. A sensor that nobody checks is a cost centre. A simple dashboard reviewed every Monday morning changes behaviour within a quarter.
